APPLICATION FORM FOR REGISTRATION – NURSERY & LKG (SESSION 2026–2027)

Instructions

1. Important Dates:

• ⮚ Application Form for registration to Nursery, LKG will be available online from Monday, 15th December 2025 to Monday, 6th January 2026.
• ⮚ Hard copy of the online filled Registration Form must be submitted to the school office with necessary documents from Monday, 05th January 2026 to Saturday, 11th January 2026, between 08:30 a.m. to 02:00 p.m. only.
• ⮚ The identification (interaction) of the child with parents will be held in the school from Monday, 13th January 2026 onwards.
• ⮚ Admission Status will be declared on Friday, 17th January 2026.
• ⮚ Selected applicants must collect their Admission Form from Saturday, 18th January 2026 to Tuesday, 22nd January 2026.
• ⮚ Parents must come for Admission Formalities from Monday, 22nd January 2026 to Saturday, 27th January 2026.
• Admission offer stands cancelled after the given dates.

2. Form Filling Instructions:

• ⮚ Forms available on www.stjohnsupsidc.in only.
• ⮚ Age for Nursery (as on 1st April 2026): 3 years (Born between 1st April 2022 to 1st April 2023).
• ⮚ Age for LKG: 4 years (Born between 1st April 2021 to 1st April 2022).
• ⮚ Fill all required information correctly.
• ⮚ No change of Name/DOB later.
• ⮚ Parents are responsible for accuracy.
• ⮚ Admission may be cancelled even after fee payment if form has errors.
• ⮚ Negligence is not acceptable.
• ⮚ DOB as per Aadhar card will not be accepted.

3. Documents Required:

• ⮚ Copy of Registration Form
• ⮚ Fee Receipt
• ⮚ Photo of child with parents
• ⮚ Birth certificate from Nagar Palika/Nagar Nigam only or other authority approved by U.P. Govt.
• ⮚ Address Proof
• ⮚ Sibling proof (latest mark sheet of St. John’s School, UPSIDC)

4. Payment Instructions:

• ⮚ Registration Fee: INR 500/- online only
• ⮚ Admission Fee (if selected):
• ⮚ Nursery: INR 17,000/-
• ⮚ LKG: INR 22,000/-
• ⮚ No correction after payment
• ⮚ Form downloadable only after payment
• ⮚ Fee and registration charges are non-refundable

5. Other Important Instructions:

• ⮚ School does not provide transport.
• ⮚ Admission Status will be visible on the dashboard only.
• ⮚ No enquiry in the office regarding admission status.
• ⮚ Check dashboard regularly.

Note: Every communication regarding admission would be displayed on admission dashboard only, so we request you to check the dashboard on the regular basis, Do not wait for any kind of SMS from school side.
